Iowa programs that pay family caregivers
Iowa pays family members through 2 Medicaid programs. Each has its own rules for who qualifies, who can be paid and what it pays.

Consumer Choices Option (CCO)
The Consumer Choices Option lets Iowa Medicaid waiver members control a set monthly budget and hire their own workers, including adult children and other relatives. Veridian Fiscal Solutions acts as the financial management service and pays the workers. Since individual CDAC ended on December 31, 2025, CCO is the main way to hire a family member directly.
$2,691/month (reported)
How it works →Consumer Directed Attendant Care (CDAC) — agency-delivered
Consumer Directed Attendant Care pays for hands-on help at home for Iowa Medicaid waiver members. After December 31, 2025, CDAC can only be delivered by an enrolled agency, not an individual provider. An agency can hire a member's adult child or other relative as the attendant and pay them a wage.

States that border Iowa
Each state writes its own rules, and they differ more than people expect — a spouse who can be paid on one side of the line often cannot be on the other.
